Non . . Sporulating Chrysospor i um : An Opportunistic Fungallnfedion in a Neutropenic Patient

چکیده
Systemic fungal infections are a major cause of morbidity and mortality in neutropenic patients. Approximately 25% of patients with leukemia have evidence of fungal infection at autopsy. The incidence of fungal infection is increasing in immunosuppressed patients especially in the hcmato-oncology patients. This is probably due to several factors including prolonged period of neutropenia, the use of broad spectrum antibacterial therapy, the increasing use of central venous access devices, and disruption of the normal mucosal barrier in patients postchemotherapy. Common fungi isolated fr01ll this group of patients are Aspergillus, Candida, Penicillium marneffei and JIIlucor. Increasingly, infections with fungus that were not previously recognised as pathogenic are being described. We present a case of systemic infection of a rare fungal organism, non-sporulating Chrysosporiu1n species in a neutropenic patient following an induction chemotherapy for relapsed acute lymphoblastic leukemia. We were unable to determine the exact genus of Chlysosporium species. To our knowledge, with the clinical findings and the significant blood cultures, this is the first reported case in Malaysia.

Pseudomembranous aspergillar tracheobronchitis in a non-neutropenic critically ill patient in the intensive care unit
Invasive aspergillosis is an important cause of morbidity and mortality among immunocompromised patients. Prolonged neutropenia is the most common risk factor. It has rarely been reported to occur in non-neutropenic critically ill patients in the intensive care unit setting. Mortality rate in this group has been reported to be as high as 92%. We report a case of tracheobronchial aspergillosis i...
متن کاملChallenges and Solution of Invasive Aspergillosis in Non-neutropenic Patients: A Review
Invasive aspergillosis (IA) is a serious opportunistic infection, which has increasingly been recognized as an emerging disease of non-neutropenic patients. In this group of patients, the diagnosis of IA can be challenging owing to the lack of specificity of symptoms, the difficulty in discriminating colonization from infection, and the lower sensitivity of microbiological and radiological test...
متن کاملBronchoalveolar lavage during neutropenic episodes: diagnostic yield and cellular pattern.
Few data are available concerning the relationship between alveolar and blood cell populations during neutropenia. We wanted to compare the value of pulmonary endoscopic procedures with lavage in neutropenic (polymorphonuclear (PMN) count < or = 1,000.mm-3) and non-neutropenic settings. We therefore, retrospectively, reviewed the results of 118 investigations for pneumonia in patients with mali...
متن کاملInfections in Cancer Patients
Cancer therapy is a dynamically evolving field. Chemotherapy and biologic agents impact the magnitude and duration of immunosuppression in the already-immunocompromised cancer hosts who are then susceptible to a broad spectrum of infectious complications ranging from mild opportunistic infections to severe, fatal neutropenic sepsis. Numerous bacterial, fungal, and viral organisms have been impl...
متن کاملCommunity-acquired Acinetobacter radioresistens bacteremia in an HIV-positive patient.
We describe the first case of community-acquired bacteremia caused by Acinetobacter radioresistens; the patient was a 32-year-old HIV-positive neutropenic woman. Ambiguous Gram staining and poor biochemical reactivity of blood culture isolates misguided early diagnosis and therapy. Bacterial identification was based on 16S rDNA sequence analysis. A. radioresistens can be considered as a cause o...
